Club Forms

UCC Clubs Associate Membership Form

Staff and Graduates of UCC, as well as students in other third-level institutions in Cork, may apply for an associate membership for a club in UCC. Please see the form for further information.

UCC Clubs Welfare Incident Report Form

Please complete this form in the case of a welfare incident occurring during a club activity.

UCC Clubs Safety Statement

This must be filled out by the 30th September and submitted to each year.

UCC Club Trip Plan Template

The Club Trip Plan should be filled out and submitted to at least three weeks before a trip (five weeks before a foreign trip).

UCC Clubs Event Plan Template

The Event Plan should be filled out and submitted to at least three weeks before the event takes place.

Student Accident Report Form

Complete this form in the case of an accident and return it to the Department of Sport and Physical Activity within 7 days of injury. Required for insurance claims.
