Staff and Graduates of UCC, as well as students in other third-level institutions in Cork, may apply for an associate membership for a club in UCC. Please see the form for further information.
Please complete this form in the case of a welfare incident occurring during a club activity.
This must be filled out by the 30th September and submitted to email@example.com each year.
The Club Trip Plan should be filled out and submitted to firstname.lastname@example.org at least three weeks before a trip (five weeks before a foreign trip).
The Event Plan should be filled out and submitted to email@example.com at least three weeks before the event takes place.
Complete this form in the case of an accident and return it to the Department of Sport and Physical Activity within 7 days of injury. Required for insurance claims.
